Periapical surgery, also known as an apicoectomy or root-end surgery, is a minor surgical procedure performed to treat infection or inflammation at the tip (apex) of a tooth’s root — typically after a root canal has not fully resolved the problem.
Procedure Steps
Local anesthesia is administered.
A small incision is made in the gum near the affected tooth.
Infected bone and tissue around the root tip are removed.
The end of the root is trimmed and sealed with a filling (retrograde filling).
The gum is sutured back in place to heal.
Outcome
When successful, periapical surgery preserves the natural tooth and prevents the need for extraction.
Types of Periapical Surgery Treatments
Apicoectomy (Root-End Resection)
The most common type of periapical surgery.
The surgeon removes the infected tissue and the tip of the root, then seals the root end with a small retrograde filling (using materials like MTA or bioceramics).
Curettage Only
In some cases, only the infected periapical tissue is removed without cutting the root tip — if the root itself is healthy.
